Where do I find the Employment Details form?  
I have updated my employment details on the website. Do I still need to submit my Employment Details form? 
I still work for the same company but have moved offices. What documents do I need to submit? 
I have changed mentors recently, what documents do I need to submit?  
 
Q: Where do I find the Employment Details form?  
 
A: The Employment Details form can be found on the forms page within this FAQ/Forms site.  
 
NB: If you are completing the practical experience program, or if you are moving to a company where your mentor will be external then you will also need to complete sections 1 and 2 of the Employment Details form

Q: I have updated my employment details on the website. Do I still need to submit my Employment Details form? 
 
A: Yes - updating your employment details online will ensure that your contact details are up to date and current, however we still require the service documents to be submitted to verify your employment details otherwise your service will not be counted towards your practical experience.

Q: I still work for the same company but have moved offices. What documents do I need to submit? 
 
A: If you have located to a different branch of the same company you will still need to provide all service documents. You also need to provide a supporting letter on company letterhead detailing the move and your new details.

Q: I have changed mentors recently, what documents do I need to submit? 
 
A: If you are not completing the Practical Experience Program then you will just need to submit sections 1 and 2 of the employment details form and provide confirmation from your mentor of the change. Notification from your mentor can be emailed directly to practicalexperience@charteredaccountants.com.au.  
 
If you are completing the Practical Experience Program, or will be changing mentors to an external mentor, then you will need to complete sections 1 and 2 of the Employment Details form. You also need to provide confirmation from your mentor of the change. Notification from your mentor can be emailed directly to practicalexperience@charteredaccountants.com.au.
