Certificate of Public Practice Application Form (PDF)
Members commencing Public Practice need to apply for a Certificate of Public Practice using this application form.

Certificate of Public Practice Upgrade Form (PDF)
Members holding a Certificate of Public Practice and increasing their level of activities and earnings will need to use this form to upgrade their CPP to the appropriate level.

Certificate of Public Practice Cancellation/Downgrade Form (PDF)
Members holding a Certificate of Public Practice and ceasing Public Practice activities or reducing their level of Public Practice activities or earnings will need to use this application form to apply for a change to the level of their CPP.

Certificate of Public Practice Exemption Request Form (PDF)
Members of The Institute who are also members of CPA Australia may apply for an exemption from holding a CPP with The Institute if they already hold one with CPA Australia. A formal application using this form needs to be made to The Institute.

Notification of Monitored Member Status form (PDF)
Members holding a statutory registration or license who do not currently meet any of the criteria in R4, regulation 702 should use this form to notify the Institute of their statutory registration/license.
